Allow a studio owner to edit the reason codes for why a member quits.

I would like to be able to edit the drop down list of reasons why a member quits. Did they use their 3-day buyers right to cancel, did they have a medical disability, did their membership expire, did they cancel and pay a termination fee or were they terminated for non-payment. I'd like to be able to get reports by reason and by date.
